Reports Australian pilot accidentally killed civilians in Iraq air strike

The Defence Department has confirmed there are fears an Australian pilot accidentally killed two civilians and injured two children, in an air strike in Iraq last year. The incident is believed to have happened during the fight against Islamic State.
